On a Windows 2003 server with File and Print Sharing enabled, I turn on
Windows Firewall and then configure the checkbox exception for File and
Printer sharing. With the firewall on, file sharing breaks. With the
firewall off, file sharing works.

What would cause the exception to be ignored this way?

Will,

The default settings of the Windows Firewall only allow File and Printer
sharing from within its own subnet. If you click "Edit" for File and Print
Sharing, you will need to click "Change Scope" and allow connections from
All Computers (or a custom list if you desire).

Nice feature. In the "Custom List" edit text, would I specify the 192.168
class B as:

192.168.0.0/255.255.0.0

?

That looks right to me.

If that doesn't fix your problem, I would look at the pfirewall.log file.
That should let you see what is being blocked to break the connection. You
might have to make sure that logging of dropped packets is enabled.
